Test results from St. Olav delayed over two months – turned out to have bone cancer – news Trøndelag

The GP sent a report of concern to the State Administrator in Trøndelag in mid-January. “Clear deviation that must be reported to the County Medical Examiner. Obvious danger to life and health,” wrote Marthe Sand Skjelbred, the GP who sent the report of concern. She says the patient was referred by her to St. Olav for an assessment of known knee problems. A new X-ray was taken there, which should have shown changes in the knee that needed further investigation, says Sand Skjelbred. – The findings of the CT examination that was taken in November gave a strong suspicion of bone cancer, but these findings were not communicated internally at St. Olav, says Sand Skjelbred. Got a cancer diagnosis after purring. It is unknown to the GP what caused the error. Sand Skjelbred did not receive a copy of the X-ray and CT results, or the note after the outpatient treatment the patient received. In January, the patient had still not received an answer to the imaging results. The patient himself purred at the hospital and called for the images. The patient was then told that he had bone cancer. The patient has now been sent on to Rikshospitalet, because they are experts in the type of cancer that the patient has been diagnosed with, says Sand Skjelbred. Part of the basis for supervision of St. Olav – Initially, it seems to be a serious incident, quite clearly. That’s what Jan Vaage, county doctor in Trøndelag, says. District Attorney Vaage confirms that the State Administrator has received a report of concern about the case. – This case has come as both a warning case and a report of concern from the GP. This is one of the issues that is the basis for there now being a local inspection of St. Olav, says Vaage. The State Trustee and the National Health Inspectorate have opened inspections of St. Olav after the introduction of the Health Platform. The background is several warnings and messages of concern related to the introduction of the Health Platform. The health platform was introduced on 11 November at St. Olav. – Here we are in a situation where there was an investigation that was carried out before the Health Platform. We do not know what has happened and what is the cause of the delay at the present time, says Vaage. Don’t know if it is related to the Health Platform The county doctor says it cannot be ruled out that the introduction of the Health Platform has played a role in the case, although it is too early to say anything about the reason for the delayed test results. – But could it have a connection with the introduction of the Health Platform? – Basically, some delay has occurred. It could be several things, it could be the training, the introduction time of the system linked to the X-ray. We don’t know, but it is one of the things that should be found out, says Vaage. – In this case, we do not know whether the Health Platform is connected to the delay, emphasizes Vaage. Hansen / news The hospital has carried out its own investigations St. Olav’s hospital writes in an e-mail to news that when St. Olav became aware of the delay in the patient’s progress, they notified the Norwegian Health Authority. At the same time, the hospital started a review of the incident to uncover the causal conditions and assess the need to implement corrective measures, writes Merete Blokkum, director of business management. It has not yet been concluded after the review. “Beyond this, we cannot comment on the patient’s progress, but the patient will be involved in the summary of the structured review and assessment of measures,” writes Blokkum. The Norwegian Health Authority has forwarded the case to the State Administrator in Trøndelag, who has sent the case back to the hospital for internal follow-up in the company, writes Blokkum. Will examine three areas In December, a patient died of a stroke at St. Olav’s hospital. The county doctor links the death to the Health Platform, although it has not yet been concluded what caused the death. Another patient was investigated for cancer, where the referral to Rikshospitalet came a month late. The State Administrator and the Norwegian Health Authority will visit the hospital during the first half of February. They will take a closer look at three selected areas of concern: Information flow Medicine handling User interface.
