On 26 December 2022, 61-year-old Tone Løvåsen and her daughter, Elin Sofie Løvåsen (28), were found dead in an apartment in Kragerø. The deaths were discovered after a report of concern had been received from a person who had not been able to make contact with one of them. According to an autopsy report, both women died of pneumonia, but at different times. Elin Sofie Løvåsen (28) and Tone Løvåsen (61). Photo: Private Has found several errors Five days before they were found dead, the women had a visit from paramedics in the home. Earlier this year, it became known that the State Administrator in Vestfold and Telemark had opened an inspection to take a closer look at what kind of follow-up the women received. Now the conclusion is clear. The Telemark hospital gave irresponsible treatment to the deceased mother, states the State Administrator in a press release. The state administrator believes that the hospital has, among other things, breached the requirement for soundness and record-keeping. – Should have been admitted In the decision, it is stated that the hospital breached the requirement for soundness in the Specialist Health Services Act when the 61-year-old woman was not admitted. The state administrator has also found inadequate guidelines for the assessment of doctors and deficiencies in record keeping. Poor communication contributed to a municipal doctor’s decision on admission not being followed, states the State Administrator. – The patient had symptoms that indicated that she should have been admitted the same day, and the health institution itself did not provide for further follow-up of the patient, says county doctor Sigmund Skei. Sigmund Skei is a county doctor in Vestfold and Telemark. It is said that we do not provide the service we should. We take that very seriously. The hospital writes on its website that its own investigations into the incident show much of the same as the State Administrator has arrived at. Telemark Hospital takes the criticism on board, says clinic manager Frank Olav Hvaal. Photo: Martin Heggholmen / news Hvaal says that technical solutions and routines that worked too poorly contributed to the patient receiving too poor treatment. The hospital believes that there has been a failure in communication with the municipality and the ambulance service, and that the system for obtaining new assessments did not work well enough. – We are working on many measures to create improvement, says Hvaal. Measures have been taken The hospital has analyzed the incident and implemented several targeted measures, the press release from the State Administrator states. Shortly after the deaths became known, the ambulance service received criticism from Magnus, who is the nephew and cousin of the deceased women. He hopes the report can lead to the hospital improving in the future. – They must take things seriously right away, not wait.
