– For more than a year now, we have had a record system delivered where the exchange of messages, both internally and externally in the system, has repeatedly failed. It threatens patient safety. This is what managing director Grethe Aasved at St. Olav’s hospital in Trondheim writes to Helseplattformen AS, according to a letter Sunnmørsposten has obtained access to. A failure in the flow of messages creates an “immediate potential risk of patient injury”, she writes. – I am referring to the letter which is complementary and shows the seriousness of the situation, says Aasved to news. The letter is dated 1 December. 182 patients affected by new error The health platform is a joint digital platform for the entire health service in central Norway. The aim when it was introduced in November last year at St. Olav’s hospital in Trondheim was to improve the flow of information and increase patient safety. But so far the problems have been queuing up. At the latest last week, it became known that 182 patients were affected by an error in certain drug lists. This may have led to patients receiving the wrong dosage of medicine. “Measures have been implemented to prevent incorrect medication, which can have serious consequences,” Aasved told Adresseavisen about the case. On Wednesday last week, it was also known that 44 radiological referrals to partners outside St. Olav’s hospital had not been received. Requesting information The Norwegian Health Authority has carried out an inspection at the hospital and highlighted three things which they believe are very serious: Locked records when used simultaneously Lack of control over waiting lists, referrals and messages to external Medicines Administration Aasved is requesting the status of the work around the internal system audit at the Health Platform. She writes that St. Olavs has no information on how far they have come, or whether sufficient resources have been set aside for auditing and correcting any deviations. – Despite repeated requests to get control of the message flow in the form of a system audit with end-to-end testing, we have not received an answer as to where you are in this process and when you expect to have full control, writes Aasved in the letter . Acting CEO of the Health Platform, Siri Beate Berg, says they have mapped risks in the flow of messages related to radiology, referral and epicrisis. They have also started a survey of medicines in the medical record system. – These surveys have revealed a need for measures that have been implemented or are being planned. We will prepare a preliminary report for this work and send it to St Olav. We will also take the initiative for a customer meeting with St. Olav’s hospital to convey the status of this, she says.
