Safety advice Shipping no. 2024/01T On 30 January 2023, the snow crab boat Hunter was on the fishing grounds in the Barents Sea, when a fisherman while setting crab stones died after falling overboard. The investigation has shown that the minimum requirements for rescue equipment are not in balance with the risk in the work being carried out. It was not safe to use the MOB boat, and there were weak or no absolute authority requirements relating to the use of rescue equipment when working on deck. It was also not possible to locate the fisherman who had fallen overboard. The Norwegian Accident Investigation Board advises the Norwegian Maritime Directorate to implement measures that ensure a balance between requirements for rescue equipment and the expected working conditions for fishing for snow crab. The investigation has shown that the shipping company’s safety management system did not function as a safety barrier as the shipping company itself did not comply with the obligations it had undertaken. The procedure descriptions had major shortcomings and risk assessments had either not been carried out or had not been sufficiently objectively carried out. The Accident Investigation Board recommends that Havøy Kystfiske AS implement safety management measures to achieve safe workplaces on board the shipping company’s ships. The investigation has shown that the Norwegian Maritime Directorate’s audits were carried out when the vessel was not operationally manned as required by the audit guidelines. The supervisory authority thus did not have sufficient grounds to uncover language problems and the crew’s lack of knowledge about the ship’s lifesaving equipment. The Norwegian Accident Investigation Board advises the Norwegian Maritime Directorate to a greater extent to follow up its own guidelines so that ships that are audited are in the operational condition that is expected during active fishing when audits are carried out on board.
