Understanding Diabetes: A Growing Global Concern

Diabetes mellitus is a chronic disease characterized by impaired metabolism, which leads to increased blood glucose levels. This excessive sugar can cause serious microvascular and cardiovascular complications, increasing the risk of damage to various organs and raising associated mortality rates. The rise in diabetes cases poses a significant challenge to public health, affecting quality of life for millions of people worldwide.

In Spain, current statistics suggest that diabetes impacts between 5% and 10% of the total population, though many cases remain undiagnosed. For every diagnosed patient, an estimated undiagnosed case exists, illustrating the epidemic nature of the disease. The prevalence of diabetes increases with age, obesity, and a sedentary lifestyle, making it imperative to understand its symptoms and diagnosis.

Recognizing Symptoms of Diabetes

The manifestations of diabetes vary depending on the type. For instance, in Type 2 diabetes, the most common form, symptoms may go unnoticed for years. Patients often experience persistent thirst, an increase in urine output, heightened appetite, skin itching, frequent infections, and cardiovascular issues. Recognizing these signs early can be crucial for effective management and treatment.

The diagnosis of diabetes is primarily established through blood glucose measurement. Normal glucose levels in healthy individuals should be below 110 mg/dl. According to the Ministry of Health of Spain, a reliable test for glucose measurement is the fasting blood glucose test. If blood glucose levels are equal to or greater than 126 mg/dl on two separate occasions, a diagnosis of diabetes is confirmed. Additionally, if a patient exhibits symptoms and has an random glucose level of 200 mg/dl or above, diabetes is also established. Another method for screening is the oral glucose tolerance test, though this is mostly reserved for pregnant women.

Different Types of Diabetes

Among the various forms of diabetes, Type 1 diabetes predominantly affects children and young adults, particularly those under 30. This condition results from an autoimmune destruction of insulin-producing beta cells in the pancreas. The absence of insulin makes it difficult for the body to regulate glucose levels, significantly increasing the risk of ketoacidosis, a serious metabolic imbalance.

Conversely, Type 2 diabetes is more prevalent and associated with aging and obesity. It is defined by a gradual resistance of body tissues to insulin. Its onset is often insidious, with symptoms that may not become apparent until the disease has progressed significantly. While previously it primarily affected individuals over 40, recent trends indicate an increase in diagnoses among younger populations.

Another common form is gestational diabetes, which is detected during pregnancy. Women diagnosed with this condition must be monitored after childbirth, as it may or may not persist after delivery. Additionally, there are rarer forms due to genetic abnormalities, pancreatic diseases, or the use of medications such as corticosteroids.

The Importance of Early Detection

Before a person is diagnosed with diabetes, they may present with prediabetes, a condition marked by altered blood glucose levels and impaired glucose tolerance. These intermediate states pose a risk for progression to diabetes and cardiovascular diseases. The Ministry of Health recommends that individuals with risk factors—such as being over 45 years of age, having a family history, obesity, hypertension, or previous gestational diabetes—should undergo regular diabetes screening.

Preventing Type 2 diabetes largely hinges on lifestyle modifications, including weight reduction in overweight individuals and the incorporation of regular physical activity. Studies suggest that engaging in 30 minutes of moderate exercise five times a week significantly reduces the likelihood of developing the disease. Currently, the prevention of Type 1 diabetes remains an area of ongoing research.

Managing Diabetes Effectively

The treatment approach for diabetes comprises a personalized diet plan, organized physical activity, prescribed medications, and healthy habits, such as avoiding tobacco use. Regular monitoring of blood glucose levels and periodic medical check-ups are crucial for managing the disease. Monitoring methods include tests for glycated hemoglobin to assess long-term glucose control, as well as screenings to detect any organ damage, like microalbuminuria, comprehensive eye exams, and evaluations of arterial pulses, alongside managing other cardiovascular risk factors.

Through awareness, education, and proactive management, diabetes can be effectively controlled, enabling those affected to lead healthier, fuller lives.
